Privacy Policy
This policy explains how Presence Direct Primary Care PLLC (“Presence DPC”, “we”, “us”) collects, uses and protects information in the Presence DPC Patient and Presence DPC Physician mobile apps and our staff web console (together, the “Apps”).
Your health information is protected health information under HIPAA. Our Notice of Privacy Practices also applies to it and describes your rights over your medical records. This policy adds detail about how the Apps work.
Information we collect
Account and profile
- Name, email address, phone number, date of birth, gender, home address, state of residence, preferred pharmacy and profile photo.
- How you sign in: email and password, Google, or Apple. With Google or Apple we receive your name and email address from that provider. We never see your Google or Apple password.
Health information you give us
- Answers to intake questionnaires, reasons for your visit, and anything you tell your physician.
- Messages, photos, videos, documents and voice notes you send in the app.
- Notes, assessments and care plans written by your physician.
Visits
- Voice and video visits happen live. The Apps do not record them. We keep a log of each call (who took part, when, and how long it lasted).
Payments
- Payments are processed by Stripe. Your card details go directly to Stripe and are never stored by us. We keep your Stripe customer and subscription identifiers and a record of each payment (amount, date, status).
Device information
- A push notification token for each device you sign in on, so we can tell you about messages, calls and updates.
- Dictation uses your device's built-in speech recognition to turn your voice into text. That service is provided by Apple or Google under their own terms.
The app asks for the camera, microphone, photos and notifications only when you use a feature that needs them. The app does not use your location.
How we use information
- To provide medical care: review your requests, communicate with you, and hold visits.
- To assign your care to a physician licensed in your state.
- To process payments and manage a subscription.
- To send notifications about your care and your account.
- To keep the Apps secure, prevent misuse, and meet our legal obligations.
We do not sell your information, show advertising, use third-party analytics or tracking, or use your information for marketing.
Who can see your information
Access is limited to the clinicians treating you and practice staff who are authorized to help with your care or account. We share information with the service providers below only as needed to run the Apps, and otherwise only as the law allows or requires, for example for treatment, payment, or when required by law.
Service providers
- Google Firebase / Google Cloud — hosting, database, file storage, sign-in and notifications.
- Stripe — payment processing.
- ZEGOCLOUD — voice and video calls, and call notifications.
- Apple and Google — sign-in (if you choose it) and delivery of push notifications.
Security
Information is encrypted in transit. Access to data is controlled by account and role, so patients see only their own records and staff see only what their role allows. No system is perfectly secure, but we work to protect your information and limit who can reach it.
Your choices and rights
- Access and correct: view and update your profile in the app, or email us.
- Notifications and permissions: turn them off at any time in your device settings.
- Medical records: request a copy or an amendment of your medical records by contacting us.
Deleting your account
You can delete your account in the app (Profile → Delete Account) or by emailing info@presencedpc.com from the email address on your account.
- Your account is closed immediately and you can no longer sign in.
- Your personal data is deleted 30 days later. Within those 30 days you can cancel by emailing us.
- Medical records of consultations you received are kept for as long as Massachusetts law requires us to keep medical records. Payment records are kept as tax law requires.
- Everything else is deleted.
More detail: Delete your account.
Children
The Apps are for adults 18 and older. We do not knowingly collect information from children.
Where the Apps are offered
The Apps are intended for residents of the United States, and information is stored in the United States.
Changes to this policy
If we change this policy, we will update the date above. If a change is significant, we will tell you in the app or by email.
